PRP Knee Injections: Do They Work — and Are They Worth the Cost?

What Is PRP?
So what is PRP, exactly? Platelet-rich plasma (PRP) is a concentrate made from your own blood. A clinician draws a small sample, spins it in a centrifuge to separate out a platelet-rich fraction, and injects that back into the joint. Does It Actually Work?
Here's the picture, and it's a real tension worth understanding.
The pooled trial data lean positive. Many meta-analyses — including recent 2024–2025 analyses — rank PRP ahead of hyaluronic acid and placebo for pain and function, with benefits often holding at 6–12 months and looking strongest in mild-to-moderate knee osteoarthritis rather than advanced, bone-on-bone joints. Some guidance favours leukocyte-poor ("pure") PRP, but studies remain inconsistent about the ideal white-cell content, platelet concentration and dose — another reason to ask a clinic exactly what preparation it uses.

But the single most rigorous placebo trial was negative. In the RESTORE trial — 288 adults with mild-to-moderate knee OA — three PRP injections did not significantly improve pain or protect cartilage compared with saline at 12 months. That neutral result, from a large, high-quality study, is the reason the picture stays unsettled: much of the positive signal comes from pooled analyses of smaller, more variable trials.
And the guidelines no longer speak with one voice. The American College of Rheumatology recommends against PRP, while the AAOS says it may improve pain and function but rates the evidence only "limited." The VA/Department of Defense guideline stops short of recommending it. In contrast, new 2026 guidance from the American Academy of Physical Medicine and Rehabilitation (AAPM&R) recommends considering PRP for selected people with mild-to-moderate knee OA who remain symptomatic despite conservative treatment. The fairest reading isn't that PRP has been rejected — it's that patient selection and treatment standardisation still matter enormously, because "PRP" isn't one standardised product.
So the fair summary: PRP can meaningfully reduce pain and improve function for some people with mild-to-moderate knee OA — but results vary with the clinic and the preparation, the highest-quality placebo trial was negative, and it isn't a guaranteed or standardised result.
How Long Does It Last, and How Many Injections?
PRP protocols range from one to three injections. Some clinicians use a single injection; others recommend two or three spaced several weeks apart. Research hasn't established one universally superior schedule, so ask why a clinic recommends its particular protocol — and whether the quoted price covers one injection or the complete course.
PRP works gradually, with any benefit generally developing over several weeks rather than days. When it helps, symptom relief is commonly reported for around 6–12 months in trials. Repeat treatment may be considered if the benefit fades, but there's no universally established maintenance schedule.

What It Costs — and Whether Insurance Covers It
This is the factor that decides it for most people. PRP commonly costs around $500–$2,500 per injection in the United States — Harvard Health cites roughly that range, while Johns Hopkins lists about $750–$850 at its own centre. Because a course often involves more than one injection, the total can climb considerably, depending on the clinic, imaging fees and preparation system.
Coverage is limited: Medicare does not generally cover PRP for musculoskeletal or joint conditions, and while private coverage varies, many plans still classify it as investigational or exclude it. Confirm directly with both the clinic and your insurer before committing.
That cost, set against evidence that's promising but not guideline-endorsed, is the crux of the "is it worth it?" question — and it's why so many people weigh PRP against cheaper, covered options first.
When Is PRP More Likely to Be Worth the Cost?
The answer to the title question depends on your situation. PRP is a more reasonable investment when:

Side Effects, Safety and Recovery
PRP's safety profile is reasonably good, largely because it uses your own blood — but it isn't risk-free. A 2025 meta-analysis found more reported complications than with saline placebo, though nearly all were mild or moderate, and the overall complication odds were similar to hyaluronic acid or corticosteroid injections.

Separately, preliminary mouse data reported in supplementary material to a 2016 BMC Research Notes paper found blood glucosamine concentrations roughly ten times higher two hours after micellar transdermal application than after oral administration.
